Preventing Checkerboarded Tiles when Playing Music

Hello, everyone. I have a 32 GB iPod Touch, which I believe is 5th Generation. If it helps to know the actual model number, it is MD723LL/A. It is running iOS 8.3 to the best of my knowledge.

May I please ask two questions?

1. I would like to disable screen rotation and keep the device in Portrait mode. I’ve done a Google search to find out how, and have read several sites, as well as watched a few You Tube videos. I’ll be darned if I can see a Rotation Lock button or anything like that. I’m wondering if the documentation I read pertained to earlier versions of the model, or operating system. How do you do this, please?

The second question is a bit more important to me.

2. Sometimes when I play music and I hold the iPod, the screen changes to what looks like a black checkerboarded series of tiles. I've tried to research this screen and haven't figured out what causes it or how to prevent it. Does anyone know how to prevent this sort of screen from appearing? I would prefer to lock the screen so the default screen that displays the album cover, title, etc. stays put.

I apologize if I haven't quite explained what the screen looks like well enough.
