iPhone six volume issue

For some reason the volume level on my six goes down on its own for no apparent reason. I have the volume turned up to a high level while playing music and the out of the blue the volume is reduced without any prompting. Oddly enough the volume meter shows the volume level is turned all the way up and is maxed out. I am running the latest version of the software. What is the problem and how do I fix it?
