Playcounts not updating

I've tried searching for this without success so I'm hoping someone has some ideas.
Sometimes when I sync my iPhone the playcounts aren't updated. Today I found I'd lost 2 weeks of playcounts.
It seems random so sorry to be vague but can anyone help?
