I've checked the forums and see some similar problems with older devices, but this happened with my iPhone 6 Plus and I'm running a fully updated iTunes 12.
So I was building a new playlist on my iPhone using iTunes when I noticed that some of the songs from one album were duplicated. I tried to remove the duplicates manually but that just ended up removing the song completely from the iPhone. I just cleared up the rest of the album, and reloaded the songs from iTunes, which was done pretty easily.
To my shock, on the phone, the cover artwork which was embedded into the music itself had vanished! When I play the music, however, the cover artwork does show up, but it doesn't appear when I view the list of songs or in cover art view when I turn the phone to landscape mode. But surprisingly it is there when I play the song. Does anyone have a solution for this? It's really bugging me because I like to keep my music organized and that blank square where the cover art should be is really bugging me.
